People often notice a problem long before it reaches the person who can fix it. The difficulty is rarely a lack of care. More often, the reporting route is unclear, inconvenient or easy to forget.

A QR code can give staff, customers and contractors a direct reporting route from an ordinary phone. They don’t need to download an app or create a reporter account.

The form should ask for enough information to understand the issue without becoming a long questionnaire. Clear categories, a short description and optional evidence are usually more useful than a form people abandon halfway through.

Making reporting easier only solves the first part. Each useful report should reach an authorised person who can review it, set a priority, assign responsibility and record what happens next.

Learn from repeated reports

One report may need a simple action. Several similar reports can point to a recurring problem, a weak process or an improvement that should be used elsewhere.
